Microprocessor Knees May Provide Benefit to Community Ambulators

LEIPZIG — A rehabilitation specialist reported that transfemoral amputees seem to benefit from microprocessor-controlled knee joints while performing activities of daily living.

Patrick Theeven, MSc, of the Adelante Centre of Expertise in Rehabilitation, presented data on the microprocessor knee joint and community ambulators at the 13th ISPO World Congress, here.

“We believe, in the case of the microprocessor knee joint, that it is not only beneficial for the active wearers,” Theeven said. “If we can improve performance in a functional environment … then this would be an important gain.”

In cases of transfemoral amputation, the knee joint plays a pivotal role in functional level. Differing features from knee to knee affect individual performance. Theeven and colleagues set out to determine the possible advantages of microprocessor knees for community ambulators.

The study involved 30 participants, ranging in age from 18 to 75 years-old. Theeven used a double cross-over design method to study the participants with the selected mechanical prostheses: the C-leg (electronic swing and stance) and the C-leg Compact (electronic stance). Participants were acclimated to each of the devices in the lab and permitted to wear at home for 1 week.

Theeven employed a new assessment tool called the ADAPT (Assessment of Daily Activities Performance in Transfemoral amputees) test which uses 17 common daily activities like navigating around toys on carpeting and getting in and out of a car. He recorded performance times with the participants wearing each of the prostheses.

Through clinical observation, Theeven and his team concluded that microprocessor-controlled knees can enhance performance for the majority of limited community ambulators.

“This is a very heterogeneous group that can be divided into at least three subclasses,” Theeven said.

Additionally, researchers found variations within this amputee classification and suggest further research to assess the true difference in functional performance levels.

With a 9-year-old bi-lateral through the knee son, this is encouraging news for him for the future.

I am a disabled Vietnam veteran and I have an above-knee amputation of my right leg and my left leg is stiff at the knee. About 6 years ago, I received my C-Leg which Medicare paid for after a lot of paper work. The leg works great for me and has saved me a lot of spills. About a year ago I had another leg made which the VA paid for. Just out of curiosity I tried to see if the VA would spring for a C-Leg. They told me that I would not be able to get a micro-processor knee unless I could pass a certain PT test given by them. I had to go outside the hospital and walk all around up and down hills at a certain speed with a person from the prosthetic clinic as my evaluator. I said if I could do all that I wouldn't need a C-Leg in the first place.The VA wouldn't budge so I never told them that I already had one for 6 years.The reason I went through Medicare in the first place is because my prosthetist told me I would almost certainly be denied.The point is a lot of older people and less ambulatory people could benefit from a micro-processor knee but If you go the the Veterans Administration, good luck. I think they turned me down just to save money on their budget. Lets face it with the on-going threat of terrorism their will be a lot of soldiers in need of prosthetics with many different degrees of amputation. I think almost all of them could benefit from a micro-processor knee.

The microprocessor knee joint has proven very beneficial to my son, a transfemoral amputee. It allows him advanced mobility and has helped restore self confidence. I look forward to each and every research advance. I do believe that the key to success begins with the therapist. The right to be mobile with the aid of a prosthesis should be a law in every state.
